SUBMISSION OF PERTINENT INFORMATION ON SCHOOL SITE AND RELEVANT SCHOOL SITE DOCUMENTS
TO: Public Schools District Supervisors
Elementary and Secondary School Principals
Head Teachers/School In-Charge
Pursuant to letter sent by the office of Atty. Ernesto D. Adobo, CESO III, Regional Executive Director of the Department of Environment and Natural Resources Region X, regarding assistance his office extends for the issuance of Presidential Proclamation for all school sites within Region X, and Unnumbered Regional Memorandum requiring this office to submit pertinent information about school sites within the Division of Gingoog City, you are hereby directed to do the following:
1. Fill up and submit the attached form to the Physical Facilities Coordinator, Mr.Edgardo V. Abanil on or before June 13, 2009 through the receiving clerk;
2. Attach photocopies of list of improvements i.e. school buildings water systems etc proofs of ownership i.e. tax declaration, deed of donation, deed of usufruct, deed of sale. whether in favor of the City Government of Gingoog, Department of Education, or Brgy Council;
3. District supervisors are further directed to see to it that the required documents are submitted on time.
